Group Liability Release Form
INSTRUCTIONS FOR GROUP RELEASES
Person signing the release for the Group must read the Agreement carefully and understand that they are signing for the Group;
If any Group participant is also a Minor, Group must provide a separate Agreement for each minor signed by his/her parent/legal Guardian.
Group must provide insurance accord stating that they have CGL insurance of at minimum 1 million for policy limits and that they have had their carrier name CMDTRA as an additional insured prior to their event taking place on CMDTRA premises.
Individual Liability Release Forms
All individuals attending events must have a signed release form. There are separate releases for Adults and Minors.
